Welcome back! By re-enrolling early you may save on your annual registration fees. (see below).

 

Download the following forms to re-enroll in Fresno Sunnyside Christian Academy. If you are new to the school, please go to our first time enrollment page.

 

2008-2009 Registration Form

Attendance Form (one for each student—even if you are doing this electronically, you will need to keep a paper record at home because computer will not retain attendance totals.)

Course of Study (one for each student—this MUST BE TURNED IN AT THE SAME TIME AS REGISTRATION)

If you have an incoming student such as a Kindergartner or First Grader, (or even an older student you’ve decided to homeschool*) please add these as well:

 

Health Forms (download each group separately) 

California School Immunization Record  (all incoming students) type this online, print it out

Report of Health Examination for School Entry (kindergarteners or 1st graders)

Waiver of Health Examination for School Entry (kindergarteners or 1st graders)

 

*In the case of an older student transferring from public school, you will need to request their records. Please fill out this Request of Records form as well (first form in this pack, do not sent other two).

 

 

 

Please mail all forms with payment. BE SURE TO MAKE CHECKS PAYABLE TO KARON RUIZ. Save 50.00 on your fee by re-enrolling early (July 15, 2008).
