YOU MUST DOWNLOAD AND SUBMIT ALL 8 GROUPS BELOW. Groups 9 & 10 are optional for high school. Do not forget to make your check payable to Karon Ruiz and not the school name. IF YOU CHOSE TO PAY HSLDA ONLINE (see pack 6), PLEASE INDICATE ON THE TOP PAGE OF YOUR REGISTRATION FORM AND BRING A COPY OF THE ONLINE RECEIPT TO THE REGISTRATION APPOINTMENT.

Pack 5: Home School Legal Defense Downloadable Application: on second page of application be sure to fill in “discount membership” bubble and provide the following information to obtain your discounted rate: Group Name: Fresno Sunnyside Christian Academy, Group Number: 299708 If you fail to do this, they will require full payment instead of the discounted rate.
